JOB TITLE: Clinical Pharmacist Acute Care LOCATION: Holy Spirit Hospital WORK SCHEDULE: Days/Evenings WORK TYPE: Full Time
JOB SUMMARY: The Clinical Pharmacist provides quality pharmaceutical care defined as being responsible for assuring the provision of safe, efficacious and cost-effective medication therapy for all patients. The Clinical Pharmacist will develop, recommend and monitor evidence based pharmacotherapy care plans for patients cared for by Geisinger with emphasis on patient outcomes and quality of life. These care plans include the assessment of patient’s medications for indication, effectiveness, safety and adherence issues as well as patient and laboratory parameter monitoring, documentation requirements and outcomes. The Clinical Pharmacist will utilize integrated and routine communication with all health care professionals as well as patients concerning medication therapy management. Also, the Clinical Pharmacist performs operational and clinical functions as well as completes projects as assigned; provides assistance and in-depth drug information to medical personnel; meets the qualifications to provide care for patients in the specific age range on their assigned clinical area. Performs duties to provide the highest level of patient care and maximize patient satisfaction. Mutually agreed upon goals and objectives will be established and reviewed on a yearly basis Integration with the pharmacy residency training program (where applicable) and mentoring of other pharmacists and pharmacy students will be considered high priorities within the strategic development plan. Works under the general direction of the Clinical Coordinator, Assistant Director of Pharmacy and the Regional Director of Pharmacy.
COMPETENCIES AND SKILLS: Demonstrates excellent communication and interpersonal skills. Demonstrates clinical and operational pharmacy practice knowledge and skills. Proven ability to work effectively in a team based environment. Requires successful completion of Geisinger certification programs and continuing to fulfill ongoing competency requirements.

EDUCATION AND/OR EXPERIENCE: Doctor of Pharmacy (PharmD) degree from an accredited college and current licensure or eligibility for licensure by the Pennsylvania State Board of Pharmacy required. In the absence of a PharmD, a Bachelor of Science in pharmacy with appropriate clinical pharmacy experience will substitute. Pharmacy Graduate Year 1 (PGY1) residency experience or equivalent clinical experience preferred. Residency Required |
