Functions as a member of the multidisciplinary treatment team for Geisinger at Home and the Division of Psychiatry. Will deliver care to patients directly in their home who are part of the Geisinger at Home program. The patients include those with six or more chronic conditions as well as patients who meet palliative care criteria. Reports to the Psychiatrist for Geisinger at Home as well as well as the Chief CRNP.
Primary job foci include:
1. Assessment, diagnosis and treatment Geisinger at Home patients: Provides direct patient care of behavioral health issues in patients’ homes which can include prescribing of medications and monitoring of symptoms. Will examine the interaction between the behavioral health and medical symptoms/concerns. Will consult with psychiatrist on the provision of care.
2. Will participate as a member of the behavioral health team which includes the psychiatrist and licensed clinical social workers. Will collaborate and coordinate care within the behavioral health treatment group. Will coordinate referrals to additional services as needed.
3. Functions as key component of Geisinger at Home Multidisciplinary Team
Attends/participates in Team meetings, encourages communication between team members and encourages participation of the patient and family in their care. Collaborates with internal and external team members.
Competencies and Skills:
Demonstrates sound knowledge of psychiatric and psychological disorders and their related treatments/medications.
Demonstrates ability to conduct basic medical examination.

Education and or Experience:
Graduate of a nurse practitioner program approved by the Pennsylvania State Board of Nursing required.
Current Pennsylvania Registered Nurse License required.
Certification from the Pennsylvania State Board of Nursing as a Nurse Practitioner required.
Experience in a professional psychiatric setting such as a psychiatric inpatient unit or psychiatric consultation or emergency service is preferred but not essential.
Working Conditions and Physical Demands:
Primary work setting is in patients’ homes. Position does not take call within Geisinger at Home.
