HopeHealth is seeking an experienced Hospice Team Physician. HopeHealth's headquarters are based in Providence, Rhode Island. Its affiliate members are Hope Hospice & Palliative Care Rhode Island, Hope Palliative Care Center, HopeHealth Massachusetts and Visiting Nurse of HopeHealth.
The Hospice Team Physician serves as the physician member of the hospice interdisciplinary team with overall responsibility for the medical component of patient care. The Medical Director works in conjunction with patient attending/primary physicians and the interdisciplinary team to care for the general medical needs of patients for palliation and management of their terminal illness and related conditions. The Medical Director reports to the Medical Director for Hospice Services.
This is a part-time position working between 24 - 40 hours per week.
Duties / Responsibilities:
Certifies and re-certifies that the patient meets the medical criteria for admission to hospice based on available diagnostic and prognostic indicators
Collaborates with the patient's attending physician
Documents care provided in the patient's clinical record
Serving as a medical resource to one or more hospice interdisciplinary team
Provides consultation and education to colleagues and students related to admission criteria for hospice and palliative care
Demonstrating knowledge in communications and counseling patient/family in dealing with end-of-life issues
Participating in resolution of interpersonal conflict and issues of clinical and ethical concern
Palliation and management of terminal illness
Prescribing medications and therapeutic regimens
Assures overall continuity of the medical services
Assures that the patient receiving appropriate measures to control uncomfortable symptoms
Participates in quality activities which support the organization's mission, philosophy, customer service and process improvement goals
May include responsibilities in or coverage for patients in HopeHealth's hospice inpatient facility
Participate in on-call coverage with colleagues
Keeps up-to-date on the latest research and trends in hospice care and pain/symptom management.
Participates in a rotation working with our Admissions and Referral Center, which includes taking admission calls and providing all necessary documentation related to admission process.
Education / Experience:
Completion of post graduate training
Board certification in Hospice and Palliative Care
Familiar with the philosophical and technical aspects of hospice and palliative care
Expertise in pain and symptom management
Ability to work as a member of a large interdisciplinary team
Excellent communication skills and experience with conflict resolution
Familiar with principles of quality management and ethical issues in hospice/home health care
Interest in and willingness to teach
Doctor of Medicine or Osteopathy degrees from an accredited medical school with a current license to practice in the State of Rhode Island
Board eligibility/certification in Hospice and Palliative Care
Must have or be eligible for membership in good standing on the medical staff of the accredited hospitals and long-term care facilities under contractual arrangements with Home & Hospice Care of Rhode Island
Valid driver's license with an automobile that is insured in accordance with state and/or organization requirements
Must be able to proficiently use computer. Knowledge and working ability of Microsoft Word, Outlook and Excel
