Supervises the intake and admission process for all Hospice Hawaii referrals
Demonstrates knowledge and shares expertise in aggressive symptom management as well as assessment of patient/family psychosocial and spiritual needs, implementing appropriate level and plan of care with team
In collaboration with medical staff, determine eligibility for hospice care
Ensures compliance with State and Federal regulations regarding the obtaining and processing of referral information and patient admissions to hospice (ie: NOE, CTI, ICD-10, etc.)
Assures documentation of all patient care activities by the admissions team is in accordance with hospice protocols
Implements protocols to assure compliance with all federal, state and local health care regulations as they affect the provision of clinical services
Participates in the after-hours on call supervisor rotation
Responsible for ensuring the strategic Business Development objectives related to growth and customer service are met
Works with liaisons in nurturing relationships and building a strong referral base
Oversee entries into the EMR/Tracker to ensure accuracy, completeness and compliance
Evaluates data on referrals and develops appropriate action plans as needed
Identifies opportunities to enhance and improve the functioning of the department including meeting productivity standards
Prepare monthly, quarterly, and annual reports to reflect department activities
Provide excellent customer service, and ensure the admissions team can do the same
Develop & implement clear strategy for scheduling and deployment of admission nurses
Ensures that day-to-day operations of the Intake and Admissions department runs efficiently and effectively
Oversees orientation of new employees and cross training of existing staff providing guidance and education
Oversees all hiring, counseling and terminations within the department
Collaborates with Chief Strategy Officer and Director of Human Resources to ensure competitive recruitment and retention activities
Collaborates closely with Community Relations Manager to ensure strong relationships with referral sources, troubleshooting any perceived problems or addressing any specific needs
Collaborates closely with Patient Care Managers to ensure smooth transition from admissions to patient care
Responsible for staff compliance with all required clinical and administrative deadlines
Works with staff on 90 day plan and participates in regular ride-a-longs with all admission staff
Responsible for annual evaluation and performance plans for admissions and intake staff
Acts as a mentor on an ongoing basis, being available for questions, support and available resources ensuring agency protocols are followed.
Maintains current working knowledge of department policies and procedures, as well as organizational compliance issues, playing an active role in education
Manages scheduling to ensure staffing meets demand; anticipates weekend/holiday needs, etc.
Evaluates staffing requirements and recommends changes in staffing patterns
Reviews and approves timesheets/time off requests
Participates in the establishment of department administrative policies and procedures
Supports agency goals
Consistently represents Hospice Hawaii to all internal and external customers with integrity and professionalism
Other duties as assigned
Generally Mondays through Fridays, 8:00 am through 4:30 pm.
Availability for 24-hour/7 day back-up on-call shared with the Patient Care Managers
May include after hours work including weekends and holidays, as needed
Knowledge and Skills Requirements:
Familiar with concepts and practices used in a hospice setting
Expertise in written and oral communication
Demonstrated ability in supervision of staff, documentation requirements, service coordination and case management, and team building.
Demonstrated leadership in nursing or other health care related organizations
Ability to develop and implement long and short range organizational goals
Ability to work harmoniously with a broad range of staff, volunteers, and the community as a representative of Hospice Hawaii
Demonstrates good judgment in seeking and accepting guidance, facilitates clear and honest communication with other team members. Demonstrates ethical conduct and judgment that is in harmony with the mission and values of Hospice Hawaii
Operates phone system, computer (Word, Excel, Microsoft Outlook, Electronic Medical Records and e-mail) pencil sharpener, calculator, copier, facsimile, and postage machines
Including, but not limited to general medical equipment such as electric bed, wheelchair, shower chair, bedside commode and lifts. Medical supplies including, but not limited to dressings, change materials, needles and syringes and Foley catheters or suction equipment
Frequent use of own personal motor vehicle which meets valid registration, safety and insurance requirements. Must possess a valid Drivers' License, current auto insurance, and reliable transportation
Physical, Mental and Communication Demands:
Requires long periods of sitting/sitting while driving
Requires use of hands and must be able to speak, read and write English. Vision abilities include close vision, depth perception, and ability to focus.
Must be able to lift and move up to 25 lbs unassisted
Must be able to work on multiple tasks simultaneously and meet multiple deadlines as well as analyze problems and adopt an effective course of action. Requires learning a number of policies and procedures, writing, arithmetic and problem solving skills
In addition to being able to fulfill the physical, mental, and communication demands listed above, must be able to follow safety instructions and demonstrate critical thinking
Current TB clearance and CPR certification
Including but not limited to: bloodborne pathogens, potentially infectious body fluids and medications
Normal hazards of traffic while operating personal motor vehicle
Experience and Educational Requirements:
Current Hawaii license as a Registered Nurse
Bachelor of Science in Nursing preferred
Management experience in a health care setting preferred
At least one year of experience in healthcare or gerontology
