Coordinates management of adult home care, hospice care, skilled nursing facility care, durable medical equipment and oxygen therapy through operationalizing contracts and serving as a planner, developer and resource to members and physicians, staff, contract and non-contract providers.
Provides leadership and direction to contract service providers.
Develops and implements systems and models of care to ensure high quality, cost-effective outcomes for programs within areas of responsibility. Defines and assesses problems; develops and implements solutions for providers, staff, and contract service providers for aggregate populations.
Provides professional consultation for complex situations to internal providers, staff, members and community service agencies.
Acts as primary liaison among contract service providers, members and Kaiser Permanente systems.
Develops and implements programs for changing organization and member needs.
Two (2) years of experience coordinating/planning or delivering appropriate care in public/home health/long-term care, skilled nursing facility, rehabilitative/acute care, or hospital discharge planning.
Bachelor's degree in a health profession, social work or closely related field.
National Provider Identifier (NPI) and Taxonomy code required at time of hire.
Working knowledge of hospital discharge planning, community home health resources, skilled nursing, hospice facilities, and Medicare/Medicaid guidelines.
Facilitation, conflict resolution and negotiating skills are required.
Appropriate licensure or certification.
Examples of preferred health profession credentials are RN, LCSW, Physical Therapist, Occupational Therapist, or similar health specialty.
