Advanced Practice Registered Nurses / Physician Assistants
The Hospice Nurse Practitioner in collaboration with the Hospice Medical Director and the Director of Patient Care Services provides advanced hospice care services to patients and their families. The Nurse Practitioner will perform all face-to-face encounters for hospice patient re-certifications, obtain medical histories, conduct physical exams and educate patients and families in the disease process.
Demonstrates knowledge and principles of accepted professional nursing care; is able to apply skills in the home environment. Communicates effectively with clients, physicians, co-workers, supervisor and the public; understands proper channels of communication with the agency. Ability to work within an interdisciplinary team setting

Master's degree in Nursing from an accredited school of nursing.
Current CA Nurse Practitioner license.
Minimum of 3 years of hospice and/or palliative care setting experience at an advance level preferred. Must possess sound judgment, organizational skills and the ability to prioritize and follow-through accurately and efficiently. Computer skills are required.
