Advanced Practice Registered Nurses / Physician Assistants
HealthEast is seeking a Hospice and Palliative Care Nurse Practitioner to join their growing team in the HealthEast Care System at all Hospitals.
Provide clinical visits, care and consultations for assigned hospice patients in all four HealthEast Hospitals and in the future, in the outpatient clinic
Performs hospice and palliative care team consults, assessments and follow-ups as needed
A fully integrated electronic medical record (Epic) is utilized
Full time opportunity
Must be AANP or ANCC Certified
Ability to obtain MN-APRN license and DEA
BLS and ACLS Certified
