Provides end of life care to the patient and works collaboratively with a multi-disciplinary team of Hospice professionals to provide quality nursing care, support and education to the end-of-life patient and their family. This position requires the ability to work within a multi-disciplinary team, be well organized, have good communication skills and strong time management and multi-tasking skills.
Monday through Friday 8a-4p, 9a-5p, or 10a-6p
Tuesday through Saturday 12noon-8p or 11a-7p
RN licensed in NH with 2 years of acute care or skilled care experience. Position required that you have the ability to lift &/or move up to 50 lbs. on a regular basis.
LPN licensed in NH with 2 years recent experience as an LPN in home health or hospice, medical/surgical in an acute or skilled long term care setting.
