VNA Health Group is seeking a Director, Performance Improvement. This position will be responsible for leading continuous improvement and change throughout the organization. Collaborates with partners/leaders/clinicians to build quality, efficiency, effectiveness and a sense of share accountability. Partners with patients to improve care. Uses measurement to improve health care quality and value.
Principal responsibilities include:
Assessment of the organization’s quality improvement capability, designs an improvement plan (aligned with strategic objectives) and executes a plan (leading change) to achieve successful outcomes.
Builds improvement capability within the organization, which is key to sustainability as the organization learns how to improve quality, value and the patient experience of care; this includes coaching and mentoring the individuals responsible for facilitating this activity in improvement science methods, tools and approaches (including process flows).
Oversees team-based improvement activity, both within and across settings, to promote teamwork for high-quality and high-value care, and to standardize and spread ideas that produce results.
Hands-on approach of meeting with stakeholders and building an integrated approach to attaining targets across the care continuum. Leading change on all levels of the organization to formalize the accountability process.
Develops a partnership with clinical leaders to focus on better care coordination, transitions of care and building a model of care delivery that enhances the patient experience and value of care.
Incorporate the patient’s “voice” in the system feedback about organizations performance. Develops and implements a plan for improvement of the patient experience, with the inclusion of patients/consumers at the design table, but at the very least beginning with the use of data as a means of guiding process improvement.
In close collaboration with the Reporting and Analytics Group, helps clinicians and teams to use data as information to guide efforts of quality care improvement. Included in this area is the identification of meaningful metrics needed to monitor performance; data analysis and trending; use of benchmarks to identify possible best practices for possible adaptation.
Master’s degree in health related discipline
Certified Professional Healthcare Quality (CPHQ) certification required
7+ years of quality and process improvement experience
Excellent verbal and written communication skills
Excellent computer skills with proficiency in MS Office products
Excellent analytical, organizational and interpersonal skills
