Join our Community Palliative Consultants (CPC) team to provide extraordinary inpatient palliative consultation services in 1 of 7 acute-care hospitals on the Atlantic coast of Northeast Florida, from historic St Augustine to metropolitan Jacksonville. CPC is a division of Community Hospice, one of the nations premier not for profit community hospice and palliative care organizations.
The ideal candidate will be a graduate of an accredited MD/DO training program, be Fellowship trained with a certification in Hospice and Palliative Care and demonstrated experience in working with Palliative Care patients. This individual has a strong work ethic to assure that our staff and patients are treated respectfully and compassionately, and will strive with us to achieve the level of excellence that has been the benchmark of the care we expect and provide.
This individual should be self-directed and one who can make recommendations and assist in formulation, dissemination and achievement of team objectives, policies and plans. The position requires commitment, engagement, initiative, innovation, and flexibility in palliative care delivery. Individuals should have strong clinical skills and remain well-informed of the newest Palliative Care treatments.

Community Palliative Consultants is a subsidiary of Community Hospice of Northeast Florida, which has served the Greater Jacksonville Metropolitan Area for 37 years, assisting nearly 1,000 patients daily and more than 6,000 patients per year. Formed in 1979, Community Hospice was the first hospice program in Northeast Florida. This hospice program provides services to 90 percent of the Greater Jacksonville, Florida, market, which includes Duval, St. Johns, Clay, Baker and Nassau counties.
