The professional home health nurse will assess human responses to disease process, illness and/or injury. The registered nurse will identify health problems and provide restorative, supportive, and preventive nursing measures to individuals under the home care program in their home by executing the medical regimen prescribed by a licensed physician. The registered nurse will identify health care needs for the patient and their families enrolled in the hospice program to provide education and an environment to assist the patient to a comfortable and dignified death.
The registered nurse is fully responsible for all actions as a licensed nurse and is accountable to clients for the quality of care delivered. The registered nurse reports to his/her designated immediate supervisor regarding work planning and assignments.
MAJOR DUTIES AND RESPONSIBILITIES:
*1. Assesses patients by collecting, analyzing data to recognize patients health problems and perceptions. Interviews patients and/or caregivers to elicit all pertinent information.
*2. Plans nursing intervention designed to teach patient health management and maintenance to return patients to optimum level of health. Supports patients and patients families with coping mechanisms when dealing with death and dying.
*3. Plans nursing intervention designed to teach patients and their caregivers and care management to primarily assist the client to a comfortable and dignified death and dying process.
*4. Implements, reviews, and revises a plan of care and nursing interventions to achieve desired outcomes. Monitors patients after implementation of care plan to assess effectiveness of care and patient response. Instructs all those involved in delivering care to ensure understanding and dealing with health problems. Directs and supervises other levels of clinical personnel in delivery of care to their primary patients (LPN, HHA) as needed. Notifies physicians of lab reports and significant changes in patients= conditions as warranted.
*5. Makes appropriate referrals to other disciplines and programs/resources and coordinates overall care. Offers community resources to patients and families.
*6. Maintains appropriate professional licensure at all times.
*7. Keeps comprehensive, problem-oriented medical records of his or her work as required. Maintains current and accurate medical records on all patients. Completes nursing clinical progress notes comprehensively. Maintains accurate and legible progress notes.
*8. Provides written and statistical reports or assignments and submits these as required.
*9. Assumes responsibility for recertification of a medical plan of treatment and submits a summarized report to the physician when needed, but at least every 60 days, for review and signature. Understand and implements the Hospice benefit election periods for the Hospice client and maintains re-certifications in conjunction with the election periods.
*9. Collaborates and effectively communicates verbally and in writing with peers, patients, supervisors, physicians, and other professionals. Maintains good working relationship with peers, supervisors, and co-workers. Accepts people in a non-judgmental manner.
*10. Plans, coordinates, and guides students, new staff, and other health care team members who are assigned to work with the team.
*11. Reports changes in patient status to supervisor as needed. Seeks and uses clinical guidelines. Participates in team/care conferences.
*12. Functions as committee member when requested and contributes professional expertise at assigned committees to provide continuous performance improvement. Participates in community health programs.
*13. Maintains confidentiality regarding information on patients, staff, and agency matters.
*14. Assumes the legal responsibility of competency within the scope of the practice of professional nursing.
*15. Distributes and uses patients supplies prudently with primary care.
16. Performs other duties as required or assigned by emergency or other operational reasons for which the employee is qualified to perform.

EDUCATION AND/OR EXPERIENCE:
Graduate of an accredited school of nursing required. Current license to practice as a registered nurse in Pennsylvania required.
Minimum of two years clinical experience and one year home health or hospice experience required. Chemotherapy experience and/or certification preferred. IV skills, venipuncture preferred. Broad medical/surgical experience preferred.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Must hold a current Pennsylvania drivers license and have a vehicle available for work.
Must be available to work evening and nights following the schedule guidelines for the full time evening/night home care and hospice position.
This position involves occupational exposure to disease and illness including, but not limited to Hepatitis B and HIV viruses. Requires HBV and TB vaccinations.
