We are looking for Admission Nurses and Primary Case Managers for our Hospice Office.
Admission RN: Conducts a thorough assessment of the client to determine client needs. Completes all agency admit paperwork and submits in a timely manner. Establishes plan of care in collaboration with physician and patient. Obtains MD orders for care, communicates patient needs and status to MD and to interdisciplinary team. Makes appropriate referrals to other disciplines and delivers skilled nursing care to the patient.
Primary Case Managers: Works with physicians in identifying and coordinating patient/family/caregiver home care needs and providing skilled supportive care to patients and their families. Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family and caregiver. Refers and coordinates other disciplines and specialties, transfers to other levels of care and services with other providers.

Possession of a license to practice as a Registered Nurse in the State of California. One year of experience as a professional nurse in a medical-surgical facility within the last three years and one to two years of recent home care/hospice experience preferred. Possession of current CPR certification.
