written certifications of eligibility for hospice care
attendance at hospice care plan meetings for case review and medication oversight
occasional rounding at our 13-room hospice inpatient facility
Conduct face to face patient evaluations and other visits to assess hospice appropriateness.
Reports to the Medical Director.
Licensed to practice medicine in North Carolina
Board certification in internal medicine, hospice and palliative medicine, family medicine or related specialty
Experience with caring for gerontology patients
A passion for end-of-life care
