Care24 Triage RN
Triage RN - 10,000 sign-on bonus available!
Shift is Monday - Friday 3:30 pm - Midnight
Full Time RN position available to provide hospice care on our Care 24 Triage team. Provide care to resolve a problem in a patient's care and/or educate the patient's caregivers on aspects of care techniques in the field. This position requires the RN to travel to the patient's home, long term care facility, hospital or wherever the patient is residing at the time.
Primary functions are to field incoming phone calls, assessing content of the call from patients, caregivers, families, facilities and determining needs based on data collected and to assign appropriate resources/coordinate care as necessary to maximize patient comfort and enhance the quality of life for the patient and family. Calls are documented in the electronic health record.
Answers phone calls and documents patient related calls into the electronic medical record.
Utilizing assessment of information obtained for each call, determines priority/urgency of call and assigns field nurse to make in person visits.
Assures all calls regarding fall at facilities receive visit assessment by field nurse
Communicates with teams via On Call notes and emails as necessary
Ensures information is only given to approved contacts
Provides support and education to callers.
Processes workflow in Homecare Home Base and makes status changes in the electronic record
Performs field nurse duties when necessary to meet the needs of patients/families
Acts as a resource for clinical staff
Performs chart reviews as assigned.
Supports and participates in individualized team members orientation and training plans.
Orders DME via Hospice Link
Troubleshoots medication needs with field nurse in regards to refills, questions, where to obtain supply, etc.
Supervisory Responsibilities: The Care24 Triage Nurse is responsible organizing the daily activities of the Care24 and Crisis Care teams and may perform supervisory visits for LPN’s needed.

Requirements: Current FL RN license required Valid driver's license required Minimum of 2 years nursing experience required
Hospice Care experience a plus! BSN preferred Bilingual English/Spanish a plus! CHPN preferred
