Assists pediatric patients living with terminal conditions and arranges for care to enhance the patient’s quality of life and counsels their family as they adjust to the diagnosis and remains with the patient from beginning to end, easing some of the family’s burden by handling the day-to-day details of the patient’s and their families psychosocial needs. Assists Hospice patients/families to meet their psychosocial needs through assessment and appropriate intervention as needed. Acts in a consultant role to the multidisciplinary hospice team. Furthers the knowledge and understanding of the social work role of Hospice through appropriate educational outreach.
REQUIRES: MSW. Current licensure as a Social Worker in the State of Hawaii. One year's previous experience. Excellent customer service and interpersonal skills with ability to be patient, maintain composure and interact with patient’s and their families with courtesy, tact and discretion. Good oral and written communication and listening skills with the ability to clearly communicate information to others. Ability to be flexible and adapt to changing situations on short notice with ease. Ability to safely operate an automobile - must have personal automobile available for business use, valid/current driver’s license and good driving record as evidenced by current traffic abstract. Excellent computer skills.
