This position is responsible for ensuring accurate and timely completion of Minimum Data Sets (MDS) for patients in a long-term care health care facility as mandated by law in partnership with nursing staff. Analyzes data sets, identifies problem areas and works with appropriate staff members to develop and implement modified care plans. Assesses long term care patient health needs. Assists in developing a comprehensive care plan, maintains the medical records and ensures documentation is present for coding. Adheres to Federal, state and local regulations and organizational policies and procedure. Assures the completion of all documentation, data entry and transmission in compliance with defined regulations and requirements to assure appropriate federal and/or state reimbursement. The position also measures quality of care and services utilizing established quality indicators and assists management in quality management and performance improvement.
Plans, organizes develops and coordinates the completion of documentation and the input of patient assessment data as required by federal and/or state regulations for reimbursement. Assures accurate transmission of data in accordance with regulations and maintains a compliance log of transmissions.
Maintains, updates and revises required documentation according to patient changes in condition, as regulated by federal and/or state guidelines. Effectively communicates and coordinates with members of the interdisciplinary team regarding appropriate functional/care level and transition to another level. Advises patients of rights regarding information transmitted.
Performs audits related to the reimbursement documentation and the quality management process. Reviews audit results and presents summary information for management utilization and review.
Assists in ensuring compliance from a risk management/regulatory perspective through early identification of clinical events or trends, including root cause analysis and risk reduction strategy. Provides data management and summary information regarding clinical decisions for benchmarking and comparative analysis.
Maintains education and updated knowledge of federal and state regulations in order to assure appropriate reimbursement. Provides education and guidance to the interdisciplinary team regarding regulations, reimbursement system, and associated documentation to assure maximum reimbursement levels.
Licensed Practical Nurse (LPN) licensure in state of practice.
Requires a proficiency level typically attained with 3 years clinical nursing or rehabilitation therapy experience. Must have good communication skills. Must have strong organization skills and be able to work independently. Knowledge of quality improvement processes with an emphasis on clinical documentation standards, auditing, and reimbursement process.
Additional related education and/or experience preferred.
