The University of Vermont Health Network Home Health & Hospice is looking for a Hospice Liaison who will be responsible for generating, managing and coordinating referrals and transitions of care from key hospitals (as well as Skilled Nursing Facilities) in the area to hospice and palliative care services. This position will play a crucial leadership role by coordinating outcomes-driven business growth. The Hospice Liaison will work with a dedicated team including an administrator, clinical management team and marketing and sales support.
Other responsibilities include:
Cultivating and maintaining productive professional relationships with referral sources
Maintaining industry knowledge of best practices and emerging trends
Providing education for health care professionals on hospice and palliative care services
Assessing every patient referred to UVMHN HHH Hospice Program and preparing patients and families for hospice admission
Coordinating immediate communication with hospital parties (hospitalists, case managers, palliative care team members, oncology, ICU, etc.) regarding new referrals and transitions of care plans consistent with patient and family goals
Preparing medical documents for transfer
