Hawai‘i Care Choices is seeking an experienced and compassionate Hospice and Palliative Care Physician to join our team. Our ideal candidate will play an integral role within our community by providing compassionate and quality end-of life care through our team philosophy of support, counseling, and education for our patients and their loved ones.

The Physician will operate under the direction of the medical director and will assume the medical director’s responsibilities and obligations in his/her absence. The physician, will be responsible for the palliation and management of the terminal illness and conditions related to the terminal illness and will work in conjunction with the patients PCP.
Primary responsibilities of this position is to:
Provide daily leadership and clinical support for hospice and palliative care teams
Actively educate, collaborate, and promote the utilization of Hawaii Care Choices with providers, resources, and community at large.
Engage hospice patients where they reside, providing palliative care consult visits with the hospital and other facilities within service area.
Take on the role of primary care provider for hospice patients without a primary care provider/attending physician.
Participate and work collaboratively with Hawaii Care Choices’ employees in the Hospice IDT meetings.
Share night and weekend call duties with other hospice and palliative care physicians for home care and the in-patient facility.

Qualified physician must have knowledge and experience in pain and symptom management.
Knowledge of the principles and practice of primary medical care.
Adequate educational and experiential level to meet the needs of the patients, care givers, and regulations established by state and federal laws to assure quality hospice care.
Advanced and ongoing training in Hospice and Palliative Medicine Physician training, diagnostic skills, knowledge in symptom management and communication skills required.
Ability to build effective relationships and build an improved culture of collaboration among healthcare professionals to meet the needs of end of life care.
Effective leadership skills and relationship maintenance attributes.
Education & Experience Requirements
Graduate of an accredited School of Medicine.
Possession and maintenance of a valid Hawai’i State Physician’s license to practice medicine in the state.
Current Federal and State of Hawai’i Controlled Substance Registration Certificate
Two years’ experience as a practicing physician with experience in pain management and palliative care
Credentialed for medical practice at contracted hospitals and nursing facilities.
Prefer previous experience working with a hospice and/or palliative care program.
Certification by the American Academy of Hospice and Palliative Medicine
Current Driver’s License.
