This position is for our Snow Hill, NC Office!
Purpose:Effectively and efficiently lead the Assisted Care Home Health staff to meet optimal clinical, quality, financial and operational outcomes and goals in pursuit of the agency mission.
Program Operations Management:
Develop standard operating procedures, in coordination with the COO and the Director of Performance Improvement and Education for the Home Health Program; review procedures on a regular basis to ensure they meet the needs of clients and the agency.
Provide resources to ensure constant quality care is provided to clients.
Act as Executive Leadership Team representative for operations of Home Health Program.
Oversee the hiring process for program (ACHH) employees to ensure the quality of care.
Assists in the development of the Home Health annual budget.
Act as the Home Health representative to the Executive Leadership Team (ELT) to provide program statistics, in coordination with the COO.
Assists Chief Executive Officer & Chief Operating Officer with the maintenance of written service agreements, including financial arrangements, with other approved providers for appropriate services necessary to meet the needs of patients. Monitors and controls all care services not furnished directly by the agency to ensure company policies and regulatory compliance are being met, for example; services furnished under contract.
Program Staff Supervision:
Promotes an environment that encourages cooperation, collaboration and creativity to advance collaborative efforts between AssistedCare Companies. Maintains and encourages open, honest communication – always focusing on retention.
Directs and oversees the duties/responsibilities of program (ACHH) employees (field, clinical, support and intake staff).
Ensures tools and educational resource information are made available to program employees.
Conducts staff meetings to communicate program information.
Provides guidance to Clinical Team Managers regarding clinical and operational processes.
Quality Care Management:
Ensures quality care standards are established, communicated and met for the Home Health Program. Proficiency with said standards, guidelines, and tools there-of.
Assures that services are provided with consideration of physical, psycho-social, cultural/spiritual, and age-specific needs of those served.
In coordination with the COO and the Director of Performance Improvement and Education, develop monitoring methods to ensure the quality of client care and utilization, regulatory compliance and identification of program inefficiencies (i.e. periodic audits, documentation, reporting, etc.).
Executive Leadership Team:
Provides reports, statistical information, and recommendations in all areas of individual responsibility, in coordination with the Director of Performance Improvement and Education.
Assists in development of a plan of action to address concerns and ensure continued program growth.
