Description Care Partner - Vanderbilt Home Care Required: High School Diploma or GED, 1 year of relevant experience Strongly Preferred: Nursing Assistant Certification JOB SUMMARY: Assist with the delivery of patient care and maintenance of unit environmental standards under the supervision of a licensed nurse. Your Role Care Partner will go to the patients home to provide the prescribed care. Private Duty positions will remain with the same patient the entire shift. Most shifts will be 10 to 12 hours.
