The Clinical Social Worker provides on site and/or home care services to participants/patients and their families maximizing utilization of personal family community and health care resources.
Actively participates in performance activities to promote efficient and effective processes of care and service.
Assesses all sources of information which validate that transitional care and service needs are being appropriately addressed by the treatment plan.
Assesses the capacity of the home caregiver to cope with the participant/patient's needs in the development of the plan of care.
Clearly documents transitional plans and counseling sessions and updates.
Demonstrates a good working knowledge of transitional facilities and agencies available.
Demonstrates a practical understanding and balance of the need to resolve participant/patients psychosocial issues while responding to the need for efficient flow to meet third party payer requirements.
Evaluates or assists the attending physician/interdisciplinary team in evaluating the psychosocial needs of the participant/patient demonstrating a clear understanding of each participant/patientï¿s½ diagnosis prognosis care needs and outcome goals.
Minimum of three years in case work or counseling in a health care setting or social delivery system.
