·Participates in team conferences, assisting in care planning and review.
·Available as on call consultant to patient team members regarding a patient's physical condition.
·Participates in patient care, providing direct patient care and evaluation for both inpatients and outpatients as required to meet patient needs or Agency needs for care planning or initiation.
·Serves as a liaison with patient's Attending Physician, and acts as Attending Physician if so elected by a hospice patient with no Attending Physician.
·Available as a consultant to physicians concerning potential patients. In the absence of an Attending Physician, acts as primary physician.
·Offers advice, and is available as consultant to nurses, on medical interventions to relieve pain and symptoms of disease.
- Provides education through teachings, inservices, new employee orientation, and other interactions to enhance the overall clinical competencies of the Agency’s clinical staff with an emphasis on hospice and palliative care.
·Provides clinical staff with the latest updates and information gathered from seminars, lectures, publications, medical journals, and other sources related to hospice and palliative care.
-Doctor of Medicine or Osteopathy degree from an accredited medical school.
-Prefer board eligibility or certification in Hospice and Palliative Care.
- Current license to practice in the state of Indiana.
- Membership in good standing on the medical staff of CHC contracted facilities, as appropriate.
-Possesses comprehensive knowledge and skills in pain and symptom control, diagnosis and evaluation of patients, medicine, oncology, pharmacology, psychological aspects of loss, and principles of hospice and home health care.
