Desert Regional Medical Center is a 385 bed acute-care hospital classified as a stroke receiving center and level 2 trauma facility with an innovative , patient centered and evidence-based Rehabilitation Services Department. Our compassionate team provides a wide range of inpatient and outpatient services, including acute care rehabilitation,joint replacement & spinal surgery, neurosurgery, ICU, Telemetry, step-down care, skilled nursing, as well as outpatient therapy, hand and lymphedema clinics.
The LVN II performs, under the direction of the Registered Nurse, provision of care of patients to whom he/she is directly assigned. A basic understanding of medical and surgical problems is necessary, in addition to knowledge of appropriate nursing interventions related to the patient for which he/she provides care. IV certification and knowledge additionally required. Access to all Patient Health Information for treatment purposes only.
DEPARTMENT SPECIFIC DUTIES:
Age Specific Responsibilities
1. Obtains information and interprets information in terms of the patientï¿½s needs.
2. Demonstrates knowledge of growth and development specific to the ages of patients normally served by the Unit.
3. Understands the range of treatment needed as appropriate to the ages of the patients normally served by the Unit.
Primary Position Responsibilities
Performs general nursing duties within scope of practice, including but not limited to:Data Collection, Documentation and Knowledge Base
1. Performs and documents accurate comprehensive and daily shift data collection per policy and standards for specific unit.
2. Demonstrates knowledge associated with specific unit disease processes.
3. Demonstrates knowledge of commonly used drugs for specific unit.
4. Aware of patients individualized plan of care including goals and interventions.
5. Is aware of which patients have existing or are high risk for skin breakdown. Provides information to the Clinical Managers and RNï¿½s that assists in the development and revision of the treatment plan.
6. Documentation addresses the treatment plan, nursing interventions and the patientï¿½s response to the intervention.
7. Accurately collects and documents data needed for the admission assessment. Discusses abnormal patient data with RN or Clinical Manager as needed.
Education of Patients
1. Provides education to patients and families based on their identified needs and limitations in learning under the supervisor or the RN or Clinical Manager.
2. Promotes learning experiences and provides educational material to patients and their families
3. Re-enforces importance of medications. Educates patient regarding type of medication ordered and administered, including possible major side effects.
4. Evaluates and documents effectiveness of all patient teaching.
5. Provides opportunities for patients and families to question, discuss, and explore their feelings about prescribed therapies/interventions/diagnosis.
6. Assures patients are adequately oriented to the unit and room.
Nursing Care, Monitoring and Medication Administration
1. Monitors patients for changes in condition or status and intervenes appropriately in a timely fashion.
2. Reports potential crisis situations to the Charge Nurse/Supervisor.
3. Maintains a safe environment in each patientï¿½s room.
4. Ensures CNA provides appropriate pressure relief measures as per the care plan.
5. Attends report at the beginning of the shift and assumes responsibility for assigned groups of patients.
6. Maintains current LVN licensure and CPR certification.
7. Maintains current IV certification.
8. Employs principles of communication, interviewing techniques, problem solving and crisis intervention when performing interventions.
9. Interventions are based on the patientï¿½s plan of care.
10. Safely and accurately administers medication based on the 5 Rights of Medication Administration; Right Patient, Right Medication, Right Dose, Right Route, and Right Time. Uses eMar, scans patient prior to administering medications.
11. Assures patients informed consent is obtained prior to the administration of medications
12. Uses appropriate interventions in all emergencies.
13. Organizes and provides care to assigned patients.
14. Performs general nursing procedures and treatments and reports significant findings to the Registered Nurse.
15. Implements appropriate interventions based upon patient need, guidelines established, and physician orders.
16. Organizes and provides care based on patient needs and/or scheduled activities of the patient for the day.
17. Demonstrates and performs IV insertion utilizing appropriate techniques.
18. Takes and records vital signs properly, when required to clarify CNA collected results. Reports significant variances to the RN, Charge Nurse and/or Physician.
19. Observes for and documents changes in patients appearance, clinical data and other delegated measures and demonstrates ability to work toward treatment plan goals.
20. Communicates changes in patient condition to the Clinical Manager or RN.
21. Documents signs and symptoms of infection. Reports findings to the RN, Charge Nurse and/or Physician.
22. Assesses and monitors IV site and solutions for appropriate administration.
23. Monitors wounds appropriately and per Hospital and Unit policies. Completes documentation as policies.
24. Obtains consent, monitors and documents per legal requirements, any medication that is psycho-active.
General Duties and Responsibilities
1. Supports changes in clinical practice and assists the Department Director implementing changes in hospital policies or practices.
2. Ensures compliance with departmental policies and procedures. Assume responsibility for knowledge and implementation of policy, procedure, and applies to practice.
3. Functions as a resource to co-workers. Acts as mentor as needed.
4. Participates in orientation of all new staff as assigned.
5. Uses self as a role model to promote health communication and safe work practices.
6. Maintains competence in nursing practice through attending in services and participation in ongoing continuing education per year.
7. Attends Annual Competencies as scheduled.
8. Participates in the Quality Improvement Program as requested by Unit Director. Is observant of other staff performance and practices, and reports deficiencies to Department Director.
9. Follows appropriate chain of command for problem resolution approaching problem solving with a positive attitude and suggestions.
10. Maintains and improves current knowledge of regulations pertaining to long term care.
11. Reports any observed unsafe conditions to appropriate personnel.
12. Utilizes standard precautions. Ensures others observed follow Standard Precautions.
13. Handles and disposes hazardous waste safely and appropriately.
14. Consistently handles oxygen safely
15. Reports malfunctioning equipment to the appropriate department timely.
16. Ensures call lights are answered timely when not actively involved in other duties. Reports patient need to the appropriate personnel.
17. Observes for, and promptly reports to the appropriate Clinical Manager, Director or Ombudsman, any observed patient abuse. Immediately intervenes appropriately if observes abuse in order to protect the patient from harm. Understands the law as it pertains to mandated reporting.
18. Participates in all Code Blue, Fire, Disaster and other emergency drill when present to ensure knows role in event of actual emergency.
19. Adheres to hospital attendance/tardiness policy.
20. Floats as needed and in turn to specified Units within contract. Only accepts assignments within scope of practice for License and competency. Immediately informs Charge RN if unable to safely provide care for any patient assigned. Offers to perform alternate duties within scope of practice and competency.
19. Attends all Ethics and other education required by the Hospital, Unit or Director.
21. Other duties as appropriately assigned by Unit Director.
Required: Possession of a current license as an LPT/LVN, issued by the State of California; Current American Heart Association BLS certification (maintain current at all times), IV Therapy Certification, issued by the State of California
