The Home Based Primary Care Staff (HBPC) Nurse is a Professional Nurse with broad-based clinical knowledge generally developed through advanced education that enables him/her to effectively implement health promotion, prevention, and management for adult or older adult residents requiring skilled Nursing interventions and case management in home and community settings. HBPC is a skilled RN in home visit for Veterans with cognitive impairments and Veterans who are mentally ill.
