The RN is responsible for coordinating home care services post discharge from acute care and clinic visits. The Staff RN participates in the daily interdiscipilary meetings to facilitate comprehensive discharge planning. The RN is a member of the VA interdisciplinary inspection team and participates in annual inspections of VA contracted facilities. The RN conducts and documents bi-monthly visits to monitor the quality of VA paid services at facilities.
