The RN Admissions Manager is a member of the clinical team responsible for the development and management of the admissions department. The admissions department includes the admissions nurses, clinicians, and hospital team social workers throughout the designated service area. Responsible for ensuring patient/families easy access to hospice and smooth, prompt transitions from referral through admissions. Responsible for ensuring admissions and access for patients, caregivers and referral sources.

Ensures the smooth running of all aspects of the admissions department in an Assigned Cornerstone service area (Lake and Sumter County).
Provides back-up assistance for other service area admissions when needed.
Ensures that Hospice information is sensitively communicated to patient/families and that they are prepared for hospice care at home or in facilities, including principles of effective symptom management.
Evaluates referrals utilizing the established guidelines to determine appropriateness for hospice. Confers with the Executive Manager of Access, Hospice Medical Director, Team Physicians, and the Vice President Patient/Family Services as appropriate.
Ensures complete follow-up of all inquiries/referrals, keeping the conversion rate at 83%, within 24 hours of referral unless there are documented, acceptable limitations.
Communicates with the Executive Manager of Access and Clinical Liaisons on service issues that arise in the course of the intake/admissions process to develop and coordinate problem-solving interventions with referral sources.
Communicates and implements systems to meet census and admission goals, while motivating staff to achieve these goals.
Follows up and resolves internal complaints and suggestions. Develops processes to improve recurring systems issues.
Ensures adequate staffing so patients can be admitted the day of referral to within 24 hours, including after hours and weekends, unless patient or caregiver refuses or delays the admission after the referral or unless there are documented appropriate limitations.
Establishes regular department staff meetings with staff groups in the department on no less than quarterly schedule.
Establishes schedule and meets individually with all assigned staff no less than twice a year for the purpose of coaching and communicating performance status including one to two times per year field supervision.
Works collaboratively with other clinical managers and the leadership team to accomplish the strategic plans of the organization.
Ensures communication and collaboration with all hospice leadership and departments.
Provides/assures hiring, orientation, coaching, evaluation and supervision of admissions and clinician staff for designated service area.
Participates on agency committees and work projects to improve admissions processes and promote admission goals.
Achieves established census goals.
Assures staff complete documentation according to professional standards and regulatory requirements and according to established agency time frames.
Participates in Clinical Manager on-call rotation as required.
Minimum two years of supervisory/management experience or equivalent required; hospice experience preferred.
Customer service and public relations experience.
Demonstrated ability to develop and manage fast-paced, comprehensive and varied departments in multiple locations.
Demonstrated ability to build a cohesive, cooperative, collaborative team and engender enthusiasm, creativity, professionalism and excellence in service.
Valid Florida driver’s license and required auto liability insurance.
