This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until June 30, 2021. The initial cut-off date for referral of eligible applications will be October 27, 2020, with subsequent cut-off dates on the 1st AND 15th of each month, referrals will be contingent on the availability of vacant positions). Eligible applications received after that date will be referred at regular intervals OR as additional vacancies occur on an as-needed basis until positions are filled. Major duties include but not limited to the following: Responsible for administering, monitoring, documenting and evaluating therapeutic interventions and regiments specific to long term and hospice/palliative care residents. Anticipates and implements preventative patient care measures, ensuring that appropriate plan of care is implemented according to the standards of regulatory agencies such as JCAHO, OSHA and the appropriate professional nursing organization. Provides guidance and education to patients and caregivers and collaborates with other professionals at all levels, including but not limited to : Social Workers, Pharmacists, Radiology Staff, Laboratory Staff and the other Nurses and Physicians on the various clinical treatment teams. Collaborates with Community Living Center (CLC) interdisciplinary team to promote resident-centered care and provide a home like environment for long term care and hospice/palliative care residents. Identifies physiological, psychological, social and environmental variable affecting the functional status of the older adult and hospice/palliative care patient. Identifies current coping mechanisms and support systems and their effectiveness. Anticipates possible discharge needs and assists patient/significant others and multidisciplinary team in achieving goals for discharge. Documents on approved VA forms/computer programs. Determines nursing diagnoses based on analysis of assessment data. Plans and directs nursing care of long term and hospice/palliative care patients admitted to the Community Living Center. Plans interventions which promote health, self-determination and dignity at the end of life. Develops teaching plans based on assessment of individual needs. Provides individualized instruction or directs staff in teaching specific content/skill to adult geriatric and hospice patients/significant others. Participates with inter-disciplinary team and families and refers to social work potential ethical dilemmas that arise in the care of the individual with complex problems associates with age and end of life cycles. Responsible for attending in-services and continuing education activities and continued progression in knowledge and skills in long term care and palliative care.
