| The primary purpose of the Licensed Vocational Nurse (LVN) is to perform assigned duties in a manner that promotes quality health care, position customer service and the maintenance of an environment committed to serving the veteran. The LVN performs within this functional statement under close supervision of the Nurse Manager or designee. Assigned tasks require basic technical competence and skill. This vacancy will remain open until filled. The first cut-off date is 11/23/2021. Additional applications will be referred as needed. A Community Living Center (CLC) is a VA Nursing Home. There are over 100 Community Living Centers across the country. Unlike many nursing homes in the past, a Community Living Center resembles "home" as much as possible. There are activities for Veterans of all ages. There are family friendly places for visiting. Veterans are invited to decorate their rooms. And, pets are allowed to visit or live in the Community Living Center. Veterans may stay for a short time or, in rare instances, for the rest of their life. It is a place where Veterans can receive nursing home level of care, which includes help with activities of daily living (e.g., bathing and getting dressed) and skilled nursing and medical care. The mission of a Community Living Center is to restore each Veteran to his or her highest level of well-being. Major duties include, but are not limited to: Performs and documents focused assessment and reports abnormal findings to the Charge Nurse, Nurse Manager and/or RN Team Leader. As the Charge Nurse, the LVN within their scope of practice, effectively assigns, manages and directs other LVNs and Nurses' Aides in the delivery of patient/resident care. The LVN demonstrates the ability to assign patients/residents care needs and the capability of the staff to meet those needs. Demonstrates a basic ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report, and record relevant patient information. Administers oxygen therapy. Administers medication to include IV medication if certified. Completes clinical reminders with appropriate consults and alerts as indicated. Ensures medications/supplies are not expired and removed from inventory. Performs delegated procedures and treatments. Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Assists in medication reconciliation. Notifies the provider of changes. Measures, records vital signs and enters them in CPRS. Reports all significant deviations to the RN and/or health care provider immediately. Applies skin tests and measures and documents results. Work Schedule: Rotating Tours of Duty based on the needs of the unit to include days, weekends, evening and night shifts. Primary shift schedules are: 7:30 a.m. - 4:00 p.m.; 3:30 p.m. - 12am; and 11:30 a.m. - 8:00 p.m. Other tours of duty that may be utilized are: 5:00 a.m. - 1:30 p.m.; 6:00 a.m. - 2:30 p.m.; 9:00 a.m. - 5:30 p.m.; and 1:00 p.m. - 9:30 p.m. Financial Discloser Report: Not required
