Providence TrinityCare Hospice is seeking a full-time Palliative Care nurse practitioner to join a comprehensive, multidisciplinary team. This position will be a fairly autonomous role for a small Palliative Care program, which is part of a larger regional program. TrinityCare is known in the community for its outstanding accomplishments and company values. Conduct meaningful care and enjoy working in the San Fernando Valley, a beautiful community within Los Angeles County.

• Monday to Friday schedule; 8-hour shift
• Outpatient care only
• Telehealth position
• CA license required, local candidates preferred
• 1 to 2 years of experience is highly preferred

Providence TrinityCare Hospice is one of the oldest hospice programs in the greater Los Angeles area, with a staff of 180 professionals who provide compassionate end-of-life care to more than 30,000 patients and families.
