Responsible for activities related to new referrals prior to agency admissions, including, but not limited,
to working with facilities related to their discharge planning.

Friday from 3p-5p and then Saturday, Sunday, and Monday approx. 9 hours per day. Hours 8am-6pm for S, S, & M
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
·Uses customer service as a priority, answers and/or directs incoming inquires about Home Care Services. Communicates, coordinates, and disseminates information in a timely and pleasant manner.
·Ensures prescribing physician meets all criteria, submits request for new physicians to be entered into the system.
·Processes referrals for Start of Care (SOC) within twenty four (24) hours of call.
·Facilitates referrals for infusion therapy. Contacts Infusion Company to coordinate financial reimbursement, medication dosage, frequency, and delivery time to patient’s home. Faxes complete referral to Infusion Company and appropriate supervisor.
·Maintains current knowledge of reimbursement guidelines. Confers with Performance Improvement staff/supervisors as needed.
·Insurance verification is complete and relayed to appropriate staff. Accepts referrals based on knowledge of accepted insurance/managed care plans that the agency participates with.
·Documents in electronic record all pertinent information received regarding patients including code status and infection control precautions.
·Ensures physician coverage for patient care.
·Ensures proper completion of insurance intake form for nurses to reference for subsequent visit.
·Submits monthly reports of referral sources data and admission statistics, including when hospital referred patients decline care.
·Functions as Patient Care Representative at Florida Hospital Waterman as requested.
·Assists with Performance Improvement functions as requested.
·Other duties as assigned.
KNOWLEDGE AND SKILLS REQUIRED:
·Knowledge regarding reimbursement guidelines for both Medicare and Private Home Care.
·Must be fluent in the English language.
KNOWLEDGE AND SKILLS PREFERRED:
·Frontline experience in Home Health Agency
·Home Health Nurse Expertise
EDUCATION AND EXPERIENCE REQUIRED:
·Graduate of School of Nursing.
·Minimum one (1) year experience required in Home Health
EDUCATION AND EXPERIENCE PREFERRED:
·Prefer two (2) years’ experience as RN or LPN.
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
·Currently licensed by the state of Florida as professional Registered Nurse or LPN with compensatory experience
