This Social Worker position is located at the Edward Hines Jr. VA Hospital in Hines, IL and is part of the VA's Great Lakes Health Care System in the Veteran's Integrated Service Network (VISN 12). The incumbent in this social work position is assigned to the Hospice unit in the CLC, and acts in a consultative capacity for the Geriatric Primary Care Clinic and Palliative Care Team Supportive . In this role, the social worker conducts psychosocial assessments, addresses Advance Directives and Organ Donation in accordance with the standards established, participates as a member of an interdisciplinary team regarding the treatment planning process and discharge planning process, completes required charting and MDS input, and provides information regarding VA benefits and eligibility to community contacts. In addition the incumbent provides patients and family members support in connection with grief and loss processes and provides limited bereavement services. Major Duties: Position requires the ability to conduct psychosocial assessments of a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. Position requires a basic knowledge of human development and behavior (physical and psychological), and the differential influences of the environment, society, and culture. Ability to work with patients and families who are experiencing a basic emotional, medical, and social problems utilizing individual, group, and family counseling skills. Has basic knowledge of medical and mental diagnoses, disabilities, and treatment procedures to include acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and basic medical terminology. Has working knowledge and skill in the use of word processing, data management, and other computer software. Ability to learn and utilize software programs in use by the VHA. Demonstrates an understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions. May have basic knowledge of veterans benefits and services relating to special programs, service connected compensation, non-service connected pension. Facilitates referrals based upon veterans needs and eligibility. Independently identifies high-risk patients and provides case management services. Incumbent provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Incumbent must possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups. Independently identifies high-risk patients and provides case management services. Establishes and maintains effective therapeutic relationships with veterans and their families. Work Schedule: Full-Time, Monday through Friday, 8:30am to 5:00pm. Financial Disclosure Report: Not required
