| Responsible for assuring timely, accurate, and complete reporting and recording of patient data and statistics through Brightree. |
| Ensures data submissions are sent to all required entities within required time frame |
| Access reports for Quality and Tracking purposes through SHP patient survey/score board |
| Audit and review patient records prior to submission to CMS |
| Manages contracts through Meditract Software Management |
| Develops monthly quality improvement data and generates monthly reports. |
| Educator of staff on changes in regulations and documentation requirements. |
| Chart auditing for timely and accurate documentation by staff. |
| Responsible for the tracking of referrals by source to include but not limited to Palliative, Hospital, and community. |
| Maintains records pertinent to personnel and operation of the Peace Hospice Program. Prepares reports, i.e., utilization, performance improvement, outcome data. |
| Record keeping for Compliance reports and Environment of Care |
| Assist with daily staffing levels to ensure safe quality care for patients |
| Performs all other duties as assigned. |
| Prepares graphs and power points for Quality and safety Reporting to management |
