The incumbent is assigned to the Nursing and Patient Care Service and will be responsible for the oversight and implementation of the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS), a standardized instrument designed to identify the functional and health care needs of residents. This individual must demonstrate a high level of clinical, managerial and leadership skills and have knowledge and adhere to VHA policies, procedures and evidenced based practice in Long Term Care. Duties and or responsibilities may include but are not limited to the following: Demonstrates expertise and guidance in applying the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS), assessment and care planning processes considering the age specific needs of the residents. Consistently conducts self in a manner that demonstrates adherence to standards of professional practice and patient care. Utilizes the eight core competencies of the High Performance Development Model (HPDM) in practice. Promotes accuracy and timely completion of the coding of the RAI-MDS. Monitors and manages the timely electronic submission of the RAI-MDS. Supports the interdisciplinary treatment team (IDT) in performing the inter-rater reliability studies to assess the accuracy of the RAI-MDS assessment. Communicates to leadership and interdisciplinary team members any changes to VHA directives/handbooks and CMS guidelines related to the RAI-MDS process. Demonstrates the ability in identifying quality of care deficiencies; guiding committee's and teams in the appropriate analysis and plan of action. Directs unit nursing staff and the IDT who are responsible for their assigned roles for MDS data entry. Demonstrate a high level of clinical, managerial and leadership skills and have knowledge and adhere to Veterans Health Administration policies. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Safeguards the resident's right to privacy by protecting and maintaining confidentiality of the resident's and staff information in monitoring, reporting and transmission of data. Responds to concerns and the care in a non-judgmental, non-discriminatory manner, respecting the values and beliefs of members of all cultures. Demonstrates leadership in the analysis of ethical issues/concerns using appropriate theoretical principles to ensure application of the RAI-MDS processes are in compliance with organizational policy, mission and values. PREFERRED EXPERIENCE: Basic Life Support (BLS Certification). Minimum two years of Long Term Care experience preferred. One year or End of Life experience preferred. Work Schedule: Day Shift: 07:30 AM - 04:00 PM With Rotation To Evening Shift: 03:30 PM - 12Midnight. Every Other Weekend & Holidays. Financial Disclosure Report: Not required.
