Advanced Practice Registered Nurses / Physician Assistants
ChristianaCare is searching for a Nurse Practitioner to join its talented Supportive and Palliative Care team working at our Cecil Campus in Elkton, MD. The selected candidate will work in the outpatient setting and inpatient consults as needed. This is a M-F position with 4 10 hr shift days.

You will help provide clinical leadership and expertise within the palliative care clinical team and identify opportunities to improve the patient and family experience of care and to improve the effectiveness of resource use.
CLINICAL DUTIES AND RESPONSIBILITIES:
Assess health status of assigned patients through complete physical examination and medical history.
Analyze patient data and formulate an individualize plan of care based on patient assessment and needs including but not limited to pain, symptom management, psychosocial distress, spiritual issues in collaboration with physicians, nursing, and other members of the interdisciplinary team.
Evaluate and monitor the effectiveness of prescribed management plan and be able to pivot as needed.
Establish priorities based on patient needs and act on these accordingly.
Order and/or interpret diagnostic testing, therapeutic modalities, or procedures, within scope of practice and credentials.
Assist patient and family in understanding of illness, prognosis, and treatment options as well as defining goals of care and document in the EMR taking into consideration cultural differences.
Utilize teaching/learning principles coupled with an expert clinical knowledge of palliative medicine to promote and provide education and counseling to the patient and/or family including teach back.
Act as a liaison for patients, families, and other health professionals to achieve the best quality of care for the patient, be able to lead patient/family meetings, and able to facilitate communication including but not limited diagnosis, treatment options, prognosis, delivery of bad news, transition from curative to hospice, goals of care, and post death options (autopsy, tissue/organ donation).
Provide coordination of care and communication across settings/disciplines to promote: a positive image, benefits of palliative medicine, and collaboration utilizing case management techniques.
EDUCATION AND EXPERIENCE REQUIREMENTS: -Masters' degree from an accredited program for graduate level nursing -Licensed or eligible for licensure as an Advanced Practice Registered Nurse in the state of Maryland and Delaware -Advanced Palliative Medicine Certification preferred; or attained within 2 years. -Prescription Authority required; DEA Registration required -BLS required
