This position is located within Community Living Center (CLC) /Geriatrics and Extended Care (GEC), Phoenix VA Healthcare System (PVAHCS), Phoenix, AZ. The Geriatric and Extended Care PA will provide high quality patient centered care to veterans under the different services such as Geriatric Medicine Outpatient Clinic, Community Living Centers, and Palliative/Hospice care service as assigned. *May select either Physician Assistant or Nurse Practitioner*. 2.1. Demonstrates consistent clinical competence in providing nursing care. 2.1.1. Uses equipment correctly and accurately interprets data. 2.1.2. Recognizes subtle changes in patient's condition and responds timely, choosing the correct interventions. 2.1.3. Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient care problems on the unit. 2.2. Initiates and implements an interdisciplinary plan of care based on assessments. 2.2.1. Performs ongoing assessment of patient condition and response to treatment. 2.2.2. Provides leadership in establishing goals, evaluating the plan of care, and in discharge planning. 2.2.3. Initiates referrals to other disciplines and/or services as appropriate. 2.2.4. Assesses the patient's learning needs, learning preference, and readiness to learn. Modifies teaching approach based on knowledge of human behavior, motivations, and reactions to situations. 3. Accurately documents in the medical record patient findings, assessments, and care provided. 3.1. Documentation is technically informative, complete, accurate, consistent, concise, and timely. 3.2. Evaluates and documents patient response to interventions and progress towards attainment of goals and expected outcomes. 3.3. Revises interdisciplinary plan of care based upon on-going patient assessment and evaluation. 3.4. Uses the Computerized Patient Record System (CPRS) and the Bar Coded Medication Administration (BCMA) software per policy. 4. Effectively functions as the charge nurse, team leader, or in other leadership role that guides and directs others who provide patient care. 4.1. Makes assignments and directs activities of other unit staff, (RNs, LPNs, students, PSAs, other team members). 4.2. Assigns patient care responsibilities based upon patient needs and competencies of staff. 4.3. Organizes and completes own assignments in a safe and timely manner, assisting team members as necessary. 4.4. Provides direction to staff for unit activities in meeting patient care needs. 4.5. Ensures that delegated tasks as assigned are completed by other staff. 4.6. Ensures the physician and/or the nurse manager/ immediate supervisor/coordinator are notified of untoward events involving patients and staff, either by assigned RN or by self. 5. Uses a team approach to identify, analyze, and resolve patient care problems that result in improved patient outcomes. 6. Demonstrates grade-appropriate and consistent critical thinking skills. 6.1. The nurse can recognize the patient has a problem. 6.2. The nurse can manage the problem safely and effectively, recognizing his/her scope of practice. 6.3. The nurse has a relative sense of urgency. 6.4. The nurse takes the right action for the right reason (del Bueno). 7. Position-specific Competency Checklist is completed annually and is rated competent in all measured areas.
