Pharmacist(Clinical Specialist)
The Oncology/Hematology Clinical Pharmacy Specialist (CPS) must be a licensed pharmacist, preferably with a Pharm.D. or equivalent experience. The incumbent will be credentialed and will function at the highest level of clinical practice working independently based on their scope of practice as a mid-level provider performing comprehensive pharmaceutical care. These activities include providing consultative/ referral services for intensive medication therapy management (MTM) services.
