The Staff Nurse Home Based Primary Care (HBPC) position practices under the direction of the HBPC Program Director. HBPC is an interdisciplinary program providing longitudinal care to veterans in their homes. The staff nurse provides direct and indirect nursing care in a self-directed manner. Major duties include but are not limited to: Assessing the physical and psychosocial needs of patients in their homes. Performing vital signs including pain assessment. Computer documentation. Performing and documenting evaluations. Outpatient venipunctures and assuring accuracy when delivering blood to the laboratories. Performing medication reconciliation, and assuring the accurate filling of medication dispensers in the home. Other duties as assigned.
