The Geriatric Medicine Physician provides administrative, clinical, and programmatic services for the Geriatric Medicine Service at the Phoenix Veterans Affairs Health Care System (PVAHCS) to ensure achievement of clinical goals and other applicable performance measures. Specifically, these duties, as directed by the Associate Chief of Staff for Geriatrics and Extended Care Services. Duties include but not limited to: Provide clinical physician services at the Outpatient Geriatric Medicine Clinic to include primary care or outpatient consultations for chronic disease management and/or symptom management, goals of care discussions, advance care planning, LSTDI notes/orders, and care coordination for veterans and their families dealing with life-limiting illnesses. Provide clinical physician services for an Inpatient Geriatric Medicine Consult Service to assure timely and effective consultation to inpatient clinicians to assist in symptom management, goals of care discussions, advance care planning, LSTDI notes/orders, and care coordination for veterans and their families dealing with life­ limiting illnesses. Work collaboratively with other clinical disciplines to provide optimal Geriatric Medicine services with particular attention to veterans with cancer, organ failure, dementia and other life limiting illnesses. Work collaboratively with community based providers to ensure that veterans and their families have timely access to hospice services. Assist the Geriatric Medicine Service to achieve its performance quality and utilization management goals. Serve as a faculty member for Banner University of Arizona-Phoenix VAHS Geriatric Medicine Fellowship Program and for residents and students from undergraduate or graduate medical training programs.
