The Togus VA Medical Center is recruiting for a MDS Coordinator for our Community Living Center. Provide leadership in delivering and improving holistic care through collaborative strategies with others and effectively manage and maintain RAI/MDS (Resident Assessment Instrument/Minimum Data Set) processes in the Community Living Center (CLC). Maintain a high level of clinical leadership skills, competent in conducting and reporting performance improvement indicators. The RAI/MDS Coordinator has knowledge of current Joint Commission Standards of Long Term Care and their application to the comprehensive RAI/MDS assessment process, and provides oversight and education for staff on the RAI/MDS process to ensure interdisciplinary plans of care for CLC residents are complete and accurate.
