This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Nurse Practitioner (NP) functions as an independent provider and oversees care provided to patients in the Acute Psychiatric Unit (APU), Residential Rehabilitation Treatment Program (RRTP), Mental Health Outpatient Clinic setting at the North Bend Community Based Outpatient Clinic (CBOC). This includes identifying the patient's biopsychosocial concerns, defining the management of these problems, and determining the need for treatment, consultation, or referral. Specific activities will include (but are not limited to): Complete History and Physical Evaluations in an outpatient setting. Prescribe psychiatric medications for a CBOC. Write orders for necessary medications, laboratory studies, imaging, and discharge orders Request consults for medical and psychiatric services as needed for continuity of care Remain available to address mental health concerns for Veterans Manage the safe medication management program, collaborating with the pharmacy and nursing Serve as reference to Patient Aligned Care Team (PACT) teams / specialty teams / Primary Care Mental Health Integration (PCMHI) teams to facilitate timely, appropriate work-up in collaboration with specialist(s) optimizing VA resources and facilitating community care as needed Serve as the point of contact for any work-up related questions from patient/family, staff (local and/or other facility), community providers Inform patient/family of process, general disease education Assure proper follow-up of abnormal findings that are not treated during visit/stay. Identify high risk patients by acuity and/or biopsychosocial issues for intensive case management Monitor activities to ensure timely multidisciplinary review and coordination of activities necessary for development of a treatment plan and necessary supportive procedures Assess and link patient to additional services when needed, i.e. alcohol treatment, nutrition, palliative care, hospice as necessary, Home Telehealth Communicate/coordinate with PACT and PCMHI teams regarding medications, patient care needs, etc. Support patient education, treatment education, awareness of steps in care Assess and treat patient for symptom management and/or refer to appropriate specialty for higher level care needs Track all treatments according to care plan, communicating with appropriate service provider on any deviations/omissions from plan Assure treatment summary and recommendations are communicated to involved services Proactively address discharge/continuity of care needs (home care, supplies, medications, etc.). Work collaboratively with the Centralized Consult Center for internal and external referrals Provide education to patient/family for medication changes, symptom management, etc. Ensure all mandatory reporting, documentation, coding, and encounters are completed timely and tracked, as appropriate Serve as a point of contact for any medicine-related questions from patient/family, staff (local and/or other VISN20 facility), and community providers Facilitate transition of care for ongoing needs Ensure provision of biopsychosocial support to patient/family Participate in quantitative and qualitative data collection to support evaluation efforts of the client population served Work directly with stakeholders to identify needs, systems issues, and communicate activities, treatments, and ongoing needs Partner with local Leadership in communicating and addressing systems issues identified by RVAHCS, VISN or regulatory agencies for timely resolution Partner with the Leadership on Regional and VISN-wide process improvement strategies Serves on local and VISN committees Work Schedule: Monday - Friday, 8:00am - 4:30pm; Telework: Not Available Virtual: This is not a virtual position.
