The Licensed Practical Nurse (LPN) serves as a member of the nursing team in any Community Living Center (CLC) house. The LPN provides specialized care to residents with multiple disease processes on a regular and recurring basis and is expected to be vigilant in recognizing changes in residents' conditions, astute in assessing the need for intervention which may be of emergent nature requiring immediate attention. This is an intermittent position, 32 hours will be worked per month. Duties may include but are not limited to: Participates in resident care conferences and contributes to the interdisciplinary team in the development, implementation and evaluation of the resident treatment plan.
