The Patient Access Nurse Coordinator is responsible for effective coordination, monitoring, and facilitation of services necessary to meet the acutely hospitalized Veterans' medical and/or health needs in the community. The coordinator also manages and facilitates appropriate transfers into VAPSHCS. Nurse Coordinators job duties, tasks, and responsibilities include (but are not limited to), the following: Acts as a resource for Veterans, families, and both community and VA partners. Meets the needs of customers while supporting VA missions including ICARE values. Communicates and treats customers, internal and external, in a courteous, tactful, and respectful manner.
