Job SummaryResponsible for the development and implementation of home care services for participants, including the coordination of personal care services, durable medical equipment needs and nursing home care.Job DutiesCoordinates the implementation of all home care services, which include nursing, physical, occupational and speech therapy, personal care services, dietary needs and durable medical equipment, to ensure that quality service is provided to meet participant needs.Ensures appropriate authorization is obtained and entered into required software programs.Conducts initial nursing assessments and revaluations of participants, including home assessments in collaboration with social worker.Presents assessments to the interdisc
