Position Summary:North Valley Family Practice Clinic which provides primary care to our community. RN Case Manager supports the clinic by assisting patient in their plan of care, doing discharge call backs after patients have been discharged from hospital and supporting the care coordination for our patients at home. Coordinate all systems/services required for an organized, multidisciplinary, patient centered care team approach, and assure quality, cost-effective care for the identified patient population.
