The Social Worker works at the Wilmington VA Medical Centers' Primary Care Clinic at the main facility or at a Community Based Outpatient Clinic (CBOC). They provides support to the Patient-Aligned Care Team (PACT) for challenging patient-care situations by addressing Veteran's bio-psychosocial needs, eliminating barriers, coordinating care across areas, providing Veteran/caregiver/family education and support, and coordinating resources to ensure comprehensiveness and avoid duplication. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
