St. Luke's Health System's Home Care team is looking for a PEDIATRIC Home Health Case Manager - RN to join our Boise area team! St. Lukes Home Health is an extension of the patients medical provider and the St. Luke's Childrens Hospital. The home health team includes RNs, PTs, OTs, SLPs, RDs, and MSW. The team works collaboratively to take care of medically complex pediatric patients of all ages. The RN works as the primary RN for the patient and case manager to coordinate all care in the home and with the team of medical specialists in the Health System. *$10k Sign On Bonus Available* Schedules are (5) 8-hr shifts/week for full-time.
