The Medical Foster Home (MFH) Program Coordinator is a social worker who is responsible for maximizing psychosocial quality of life issues in order to assist veterans and oversees program development, case management, development of policies and care processes and coordination with HBPC interdisciplinary treatment teams. This position is located at the Carl Vinson VA Medical Center in Dublin, Georgia. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Proficiency: Must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3. Education: Must have a Master's Degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE) (transcript required). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a Master of Social Work. Licensure: Must have a full, current, and unrestricted license to practice social work in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. Certification: Must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Grade Determinations for the GS-12 grade level: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills, judgement and at least two years in an area of specialized social work practice. Candidates must be licensed or certified by a state at the advance practice level and have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Physical Requirements: Work is performed in a clinical setting that exposes the incumbent to communicable diseases common to the client population. Some work is performed outside the medical center campus, where the environment cannot be controlled. ["Coordinates the overall functions of the Medical Foster Home Program including program development, recruitment and placement of veteran residents, and development of foster homes. Establishes intake processes, oversight responsibilities and maintains a record keeping system for the program. Assists veterans to remain in their home or in a less restrictive level of care other than a nursing home. Serves veterans who have a serious, frequent and severe crises, lack of family or adequate community support networks. Promotes the model of care and reaches out to community groups and agencies to promote the foster care program. Serves as a mentor to other VA Medical Foster Home program coordinators, social workers, and other interested health care providers. Provides training to caregivers, students, residents, interns, and fellows from other disciplines regarding the advantages and disadvantages of a medical foster home. Receives and completes consults from interdisciplinary teams and other social workers on complex, difficult cases, using advance practice skills and expertise. Provides consultation to referral sources and is responsible for development and maintenance of professional standards of practice and educating other regarding such. Documents independently in the medical records utilizing paper and/or electronic formats, including psychosocial assessments, diagnostics, treatment, progress notes, follow up and referrals. Develops an assessment of the veteran in collaboration with interdisciplinary treatment teams, the veteran, family members and significant others. Reviews all data, subjective and objective and makes a clinical assessment, identifying needs and strengths utilizing advanced clinical training, insight and experience to interpret data. Established a continuing relationship with the foster home caregiver and veteran, and involved family members, evaluating progress towards goals and adjusting the treatment plan as appropriate. Acts as an advocate when it serves the best interest of the veteran and their family.
