Job Summary
Under the responsibility and supervision of the physician, gathers data and provides care necessary for implementing the therapeutic plan of the physician. Coordinates clinic visits, maintains responsibility for all aspects of inpatient and outpatient care, and performs clinical research. Serves as a patient advocate in the medical care system serving as confidante, advisor, and liaison between patient/family, physicians and multi-disciplinary team.
Job Duties
- Develops and implements tools to evaluate patient response to various programs and their components.
- Evaluates responses for trends and recommends program changes as needed.
- Develops educational information for specified problems for distribution to patients.
- Keeps said materials up-to-date.
- Works with the surgeons to come to consensus on operative and post-operative protocols.
- In conjunction with nurse educators and managers, educates nursing staff regarding protocols and expectations of working with specified patient population.
- Develops discharge plans and ensures a smooth transition from hospital to discharge destination.
- Acts as the primary contact for patients with pre-operative and post-operative questions and problems by triaging and interacting with the department administrative assistant.
- Negotiates, procures, and coordinates resources to achieve quality and cost-effective outcomes.
- Participates in quality and patient safety improvement activities.
- Assuming responsibility for identifying processes or systems that could potentially lead to errors and adverse events; knowing and following organizational and departmental policies and procedures applicable to assigned duties; participating in required organizational and departmental patient safety education programs and other activities designed to improve departmental and organizational patient safety; promptly reports serious events and incidents in accordance with established hospital policy and procedure.
- Participates in department quality improvement initiatives.
- Makes referrals to home health agencies and follows-up as necessary to ensure plan of care and renewal or updates in physician orders.
- Reviews and formulates information for the electronic medical record system.
- Assists in data collection and qualitative and quantitative research done in cooperation with the physician and the department.
- Consults with the physician, mid-level and residents on patient progress toward the expected patient outcomes.
- Identifies personal learning needs and keeps current in all aspects of specified surgery through reading literature and attending conferences.
- Participates in telephone triage as needed.

Position Details
- Hours: Day Shift; 8a to 4:30p
- Travel: Yes, regional travel on select days as needed to support team in region
- Preferred Experience: Sports medicine or ortho experience is preferred but not required.

Education
Graduate from Specialty Training Program-Nursing (Required), Bachelor's Degree-Nursing (Preferred)

Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania; Basic Life Support Certification - Default Issuing Body
