$20,000 Sign-On Bonus available!Position Summary & Mission:Greater Lakes PACT program focuses on individual's recovery, safety in the community, the promotion of wellness and independence of the individual in their residential structured setting, and the re-establishment to a higher level of social life functioning. These services are designed to support and rehabilitate individuals who are experiencing symptoms of severe mental illness that impact their ability to live in the community.The RN provides psychiatric and nursing services to clients enrolled in PACT, including medication administration and monitoring as well as general nursing care and client education. Collaborates with internal and external medication prescribers and other clinical staff to ensure effective client care. Most services are provided in the community via outreach. Monitors physical and mental status and provide nursing services to clients enrolled in PACT based on the client's expressed needs and treatment goals. 1.1. Develops understanding of each individual's strengths and challenges to assist them in reaching their recovery goals. 1.2. Uses knowledge of mental illness and the recovery model to provide effective support in meeting their recovery goals. 1.3. Engages each client in discussion to learn about their needs for general medical, nutritional, and self-care education. 1.4. Encourages client input and feedback in all interactions. 1.5. Helps clients understand the possible impact of their choices as they develop effective problem-solving and decision-making skills.1.6. Provides on-going assessment of client's physical and mental health status. Includes taking vital signs, assessing pain, providing wound and skin care, monitoring intake and output as prescribed, and client response to medications and other treatments. 1.7. Reviews lab results, and report any critical values to the Prescriber.1.8. Remains alert for any and all changes in each client's physical and mental status, and reports changes promptly to Prescriber. 1.9. Contributes to effective Treatment and Discharge Plans regarding client medical conditions and issues.2. Provides ongoing administration and monitoring of medication for clients, following established protocols.2.1. Provides effective education about mental health symptoms and medications. 2.2. Assures there are current valid medication orders for all clients.2.3. Follows all procedures as outlined in the Medication Observation Documentation manual and Medical Policies and Procedures manual.2.4. Assures compliance with all infection control protocols.2.5. Assists clients with wound care and health hygiene issues.3. Collaborates with all other treatment providers, internally and externally, to help assure effective client care. This may include prescribers, pharmacies, primary medical providers, and families of clients, along with PACT clients and staff.3.1. Works with the clinical team to plan and implement relevant treatment for clients. 3.2. Participates actively in daily team meetings.3.3. Participates in PACT staff meetings.4. Assists other PACT staff with general client care management throughout the shift as needed, including participating in crisis intervention and management of client emergencies.4.1. Demonstrates effective de-escalation skills to help clients work through and learn from their experiences.4.2. Exhibits proper role awareness and maintain professional boundaries. 5. Completes all clinical and administrative paperwork in a timely and appropriate manner.5.1. Follows charting protocol for clients, documenting health status and progress on general well-being. 5.2. Updates Medication Administration Records in a timely and accurate manner for all clients as orders change.5.3. Completes administrative documentation in a timely and accurate manner, such as Time Sheets, Time-Off Request forms, Mileage Reimbursement forms, etc.5.4. Provides all required annual documentation to Human Resources and complete all required training in a timely manner.5.5. Performs other duties as assigned.Job Scope: This position involves work of moderate variety and complexity, functioning within well-defined procedures. The position has no direct budgetary accountability. The impact of errors could be very significant if medication or other nursing care is administered improperly.Communication/Customer Contacts: This position involves a high frequency of communication, in various forms, verbal and written, with clients, other staff, and community partners. Must communicate on a regular and timely basis with supervisors, other staff, consumers, their families and community supports, and with professionals outside the agency. Must also abide by all confidentiality laws and pertinent codes of conduct. Use the computer system to send and receive email, to write reports, to schedule and complete clinical transactions. Utilizes voice mail and email on a regular basis, and responds to those contacts in a timely manner. Knowledge and Skills: Familiarity with the DSM-V, common psychotropic medications, along with a good u nderstanding of other common medications. Knowledge of the major components of a mental status examination and competency in performing on-going assessments Ability to develop and implement treatment strategies tailored to individual client needs Knowledge of community resources Well-developed skills in managing time, priorities, and paperwork; excellent written and oral communication skills Must be able to work effectively in a team environment and the ability to collaborate effectively with external partners Understanding of the importance of clear, professional boundaries, and maintenance of such Cultural competencyEducation and/or Experience: Requires a BSN degree or RN certification and a minimum of two years relevant work experience working with individuals with acute mental illness, or working in a nursing home setting, or other residential nursing care facility where clients with mental illness reside. Prefer experience working in an inpatient psychiatric facility. Must maintain current CPR/First aid certification and updated TB results (annually). Position Type: Full-Time Regular
