Your day.
As a Care Coordination Specialist / Medical Assistant, you need to know how to:
- Assist patients through the healthcare system by acting as a patient advocate and navigator. Facilitate patient education on health and disease through physician-driven protocols and/or approved printed materials. Coordinate patient care continuity with patients and families following hospital discharge and ER visits. Assist patients to sign up for medication refills.
- Coordinate referrals to SCL Health programs and external healthcare organizations and facilities using appropriate resources as necessary. Coordinate community referrals, including benefit-specific resources available to the patient such as transportation, meals on wheels, social referrals, and palliative care. Support Medical Assistants with referrals to other providers, DME, and oxygen, as needed.
- Assist the office manager with pre-visit planning management. Work with Office Manager and/or Site Lead to increase value of the morning huddle for MA staff and providers, and monitor compliance. Provide Health Maintenance oversight to the MA staff.
- Monitor care gaps and coding opportunity reports and reach out to patients to ensure prevention, screening, and chronic conditions coding is aligned with actual condition of the patients. Act as a resource for the office staff in understanding and addressing care gaps, Risk Adjusted Factors, and Hierarchical Chronic Condition codes.
- Work with providers and office manager on a process to include a discussion with an RN (when available) prior to sending patients to the Emergency Room, including prioritizing appointments for patients at risk for readmission.
- Participate in all team-based care meetings and quality improvement initiatives.
- Oversee transition coding reports and other indirect codes and opportunities, as available. Participate on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the team-based care.
Your experience.

- High School Diploma or equivalent
- Completion of a Medical Assistant training program
- Certification as a Medical Assistant or the ability to obtain within one (1) year of hire
- Minimum of three (3) years of experience in a setting serving the same age/type of patient population served by this facility or department
- Basic understanding of the referral process to chronic care management programs
Preferred:
- Graduate of an accredited Medical Assistant program
- Current Certified Medical Assistant (CMA)
- Referral management experience
- At least five (5) years of experience in a setting serving the same age/type of patient population served by this facility or department
